1 The patient has been suffering from severe tinnitus for about 6 months and above.

2 All medical treatment done did not work and the doctors have given up giving medication

3 All conservative treatments like drug therapy, dietary modifications, reassurance and counseling failed and the patient did not get any relief from tinnitus.

II. It is not medically necessary for the patient to take more than 10 TENS session every year for treatment of tinnitus, since there is no exact evidence to prove taking TENS treatment frequently gives clinical benefits for ringing in the ears.

III. Experts consider tinnitus instruments like hearing aids, maskers, and combination of maskers and hearing aids as invasive management in the treatment of tinnitus since no proper statistics has not been produced regarding the long term effectiveness of the instruments in the cure of ringing of ears. These instruments have not been approved by FDA and have been classified as class III devices. Tinnitus masking is not accepted for coverage by CMS.
