Diagnosis of esophageal cancer is done by analyzing the symptoms and medical history of the sufferer including information regarding symptoms like unintentional weight loss, difficulty to swallow foods, current medical conditions, physical examination, checking if the person is habituated to use tobacco, alcohol or GERD and tests or medical examination.

Since esophageal cancer has the tendency of spreading to lymph nodes first, physical examination of the lymph nodes including palpitation for checking the enlargement is done, biopsy or needle aspiration might also be done.

Blood tests for esophageal cancer can include liver function test and complete blood count tests. Complete blood count or CBC is conducted to know if the sufferer has anemia and the liver functioning tests are done for detecting any abnormalities and liver metastasis because of high alcohol consumption.

Imaging diagnostic tests are used for detecting esophageal cancer might include barium swallow (double contrast) test and chest x-ray. When conducting barium swallow test the patient is asked to drink a solution which contains barium. Since barium is a denser liquid which is seen in white color on the x-rays. Air is then blown inside the esophagus so that the liquid is coated o the organ's wall thoroughly. If the barium coat appears uneven on the esophagus it indicates the presence of tumors in the esophagus.

In order to test the bone metastasis of esophageal cancer the bone scan is done. Bronchoscopy is done for determining cancer which has invaded the bronchi or trachea and is normally done after giving sedation to the patient. CT scan is done to see the actual condition of the esophagus, endoscopic ultrasound is used for determining the tumor's size and the proportion of the cancer spread in the nearby tissues. PET or positron emission tomography is used for detecting the disease's staging and for detecting the metastasis.

Esophagoscopy is performed normally when there is a suspicion of esophageal cancer. When doing this test a camera with a light in a thin size tube is sent through the esophagus enabling the physician to see the mucosa and for detecting the abnormalities. Sometimes smaller sized instruments might also be passed via the endoscope for removing the tissue samples for biopsy or microscopic examination. Diagnostic tests for esophageal cancer are also done for determining the disease's stage. After this prognosis or the recovery chances are analyzed and accordingly the treatment option is chosen.
